At Tracy Community Connections Center (TCCC), our entire team is here to offer hope, support, and resources to anyone who’s experiencing homelessness in Tracy, CA, or its surrounding areas. Our mission is to provide essential services that help individuals regain stability, dignity, and independence. So whether you need access to basic necessities or more comprehensive support like housing and healthcare, our team is always ready and willing to assist you.

Our Services
Case Management
We provide personalized case management to address your specific needs. Our dedicated case managers will work with you one-on-one to connect you with the services and resources you need. Whether you’re seeking help with:
- Obtaining a new ID
- Applying for healthcare
- Accessing food stamps
- Mental and behavioral health referrals
- Addiction recovery services
- Housing navigation
- Rental and utility assistance
- Emergency housing
- Food, clothing, and household items
We’re here to support you every step of the way, offering not just practical help but also a listening and compassionate ear.
Shower Program
We understand the importance of basic hygiene and offer a warm shower program three times a week. In addition to showers, we provide hygiene products, food, and clothing to anyone in need.
- Hours: Monday, Wednesday, and Friday from 9 am to 2 pm
- Location: New Heart Church, 1647 Bessie Ave.
Street Outreach
Our street outreach team works directly in the community, visiting encampments and other locations where unhoused individuals may reside. We connect with individuals on a daily basis, assessing their needs in real-time and providing immediate assistance, whether it’s access to resources or connecting them with our case management team.
Recuperative Care
For those recovering from illness or injury, our recuperative care services provide critical support. In partnership with local hospitals, we offer transportation to and from medical appointments and provide emergency housing after hospital discharge. Our goal is to help individuals recover in a safe and supportive environment.
